FRANCESVILLE - Dr. Sam W. Heltzel, of Lafayette (formerly of LaPorte) passed away at 9:33 a.m., Monday, Aug. 19, at St. Elizabeth Hospital East in Lafayette.

He was born on March 3, 1938 in Lafayette to the late Herbert H. and Florence L. Huff Heltzel.

Survivors include:

Mark B. Heltzel, Indianapolis, Son
Matthew B. (Pamela) Heltzel, West Lafayette, Son
Jenna Heltzel, Granddaughter
Nicholas Heltzel, Grandson
Hunter Heltzel, Grandson

Dr. Heltzel grew up in Monon, and graduated from Monon High School and Purdue University. He owned and operated a family dental practice in Laporte for 47 years. He retired in 2010.

Private burial will be at Monon Cemetery, Monon.

Memorial donatiions may be given to the American Cancer Society.

Cremation arrangements have been entrusted to Frain Mortuary Querry-Ulbricht Chapel, Francesville.
